My health meter replenishes itself completely if I wait in a safe area for a while, but my focus meter only refills a little bit and then stops. The best thing to do is to play through roughly half of the game on the normal difficulty setting and then go back and play the dream sequence mission again after you become better with the controls and fighting maneuvers. Chances are, because this is the first level of the game, you're not yet familiar with the game and it's fighting controls. In order to unlock this difficulty level, you'll have to defeat every wave of troops and every other enemy that comes out of the elevator, including the final battle with Agent Smith. I can't unlock The One (hard) difficulty level in the first level which takes place in Neo's dream.
Neither one will necessarily give you an extra edge in the following level and also, you'll be able to unlock the other one later in the game, so just choose the one that you feel works best for you.

For example after the first part of the Double Cross mission you can choose to upgrade either the Weapon Strip or the Off Wall Strike moves. The shotgun is pretty much useless at long range, but can kill almost any enemy in one or two hits at short or point blank range. The assault rifle has a high rate of fire, is very accurate at long distances and inflicts incredible damage. When it comes to guns, your best bets are the assault rifle and the shotgun.
Both of these weapons inflict serious damage and do not break easy. The best melee weapons you'll find are the broad sword which can be found in the Merovingian's main hall, among other places, and the steel girder which can be found in later levels. What are the next best weapons that appear more frequently? OK, but you don't get to use the grenade launcher that much. Be sure to use it at medium to long ranges, however because it can do some serious damage to Neo if he's too close to the grenade's impact. For this reason, you should usually try to save it for the bosses of various missions. It can take out large clusters of SWAT troops, is very damaging to helicopters, and can even do some significant damage against Agents and Exiles. Generally speaking, the weapon that inflicts the most damage in The Matrix: Path of Neo is the grenade launcher and it is likely because of this, that you have access to it in only a few different missions.
